On January 19, 2024, the CFPC held the Patient’s Medical Home (PMH) Symposium in Dartmouth, Nova Scotia. Representatives from all four of the Atlantic Provinces attended. Each province shared work concerning the development of the PMH model of team-based care.
There were many successes shared. For example, PEI has implemented the following:
- Effective two-way communication and collaboration between The PEI Department of Health and Wellness, Heath PEI, and other stakeholders.
- The PMH model is being used as the blueprint.
- Future success will be measured by using patient attachment to a collaborative practice as the metric.
- Practice facilitators guide the development of effective collaborative teams.
This Symposium inspired further collaboration and sharing among the Atlantic Provinces and future ongoing discussions are planned.
